I've been looking at this again: could it be from Greece or the Balkan region?

The additional hook is very interesting, for they seem not to be too heavy to need a suporting hook to be worn!
I saw a similar design and construction in the "Bir-Collection" on p. 24 (yeah, my favourite book at the moment:): a pair earrings from Greece. They also have additional hooks (simpler, though). This one here however, seems more like a hook that is used to fix the ornament to clothing or a headband as Nada suggested, as it seems to be quite spikey.
OHhhh, of course, i was commenting on the base that it was an earring!!
Of course if it is a simple pendant then, the hook seems normal here!

Thank you Alaa eddine and Betty for your thoughts. Very helpful. It is quite heavy and the loop does not open, plus the hook is quite sharp (too sharp to fit in ear lobe).
I believe it is Ottoman, perhaps Bulgarian? I am betting it is a temporal / headdress decoration. I have a chin-chain from Bethlehem that has a similarly twisted jump-ring between two elements that I will post photos of soon.
