Maybe this is a part of a similar necklace? It is possible to open the case on one side if you take off the metal silver bar (this is not easy to do).

I think you can find these types of amuletcontainers in several Islamic countries. I have a picture of an amuletholder like yours (without dangles ) with the provenance: Iran or Afghanistan.. The chain and dangles on my necklace are more common in the Kurdistan area.
